Fully valid passport/travel document (Photocopies are not acceptable). Visa can be affixed only to a spare visa page inside the passport or
travel document.

One completed visa application form for each applicant holding a separate passport or being included on a joint passport.

*** Children under 16 years old who are included in the passport of their parent/guardian, are not required to fill out a separate form

Should an applicant fail to submit the above-mentioned paperwork and the requirements, applications will not be accepted for
consideration and will be immediately returned to the person concerned.

By post

*** Application  form(s) will be sent by post ONLY if the self addressed prepaid envelope is enclosed

Personally

Through travel or visa services agents.

* All couriers must be instructed to call and wait for the receipt which then should be delivered/reported to the sender (the receipt states the
reference number, the date of collection and the fees charged).

By Post

* A self addressed prepaid return envelope must be enclosed to the application. Recorded/Registered postal services are strongly recommended
 as the Visa Section will not accept any responsibility for loss of passports.

** Two separate Company Cheques/Postal Orders should be enclosed: the first covering the Handling Charge (10.00 GBP) and the second
covering visa information (see information enclosed)

*** Please note that if the application is submitted by post the visa procedure will be 10 working days and processing time does not include
postage.

**** If the application for visa is submitted by post it will only be returned to the applicant through the post.

Business visa
*******************

Single/double entry business visa: each application form should be accompanied by a letter of invitation from the relevant Ukrainian
company or organization (a faxed copy may be accepted). The invitation should state full name of the applicant and the purpose of journey. The
letter should state the nature and purpose of business conducted in Ukraine, contain the details of the Ukrainian business partner involved and also
quote the number of the registration certificate issued by the relevent Ukrainian authorities;
Multiple entry business visa: it is obligatory to present a detailed letter of invitation from Ukraine together with a detailed letter from the
applicants employer. Both letters should state the nature and purpose of business conducted in Ukraine and explain the necessity of issuing a multiple
entry visa. Copy of a registration certificate of a company or representative office in Ukraine should be enclosed. Technical assistance applications
should be accompanied by a letter from the Ministry of Economy, which is responsible for registration of all technical assistance projects.

*** Nationals of the EU countries, USA, Canada and Japan, Switzerland, Slovak Republic, Turkish Republic who apply for business, private,
sporting and cultural visas do not require a letter of invitation from Ukraine. It is obligatory to give answers on every question in the application
form. Application forms should be fully completed otherwise it will be returned to the applicant.

Tourist visa
******************
Single/double entry tourist visa: each application form should be accompanied by a tourist voucher with a confirmation of hotel booking received
from a hotel in Ukraine. The confirmation letter should include the full name of a person and the period for which booking was made (a faxed copy
may be accepted)

Private Visa
*******************
Single/double : each application form should be accompanied by a letter of invitation issued by the Passport and Immigration
Department of a local Police Station in Ukraine (a faxed copy may be accepted);

*** Nationals of the EU countries, USA, Canada and Japan, Switzerland, Slovak Republic, Turkish Republic who apply for business, private,
sporting and cultural visas do not require a letter of invitation from Ukraine. It is obligatory to give answers on every question in the application
form. Application forms should be fully completed otherwise it will be returned to the applicant.

Permanent Residence visa

Single entry visa: each application form should be accompanied by original Permission for Permanent Residence in Ukraine, issued by a local
governmental executive body;

Employment Visa
**************************
Single entry employment visa: each application form should be accompanied by the Work Permit, issued by the Ministry of Employment of Ukraine;

Student Visa
******************
Single entry student visa: each application form should be accompanied by the original letter of invitation from the University/College or by a letter from
the Ministry of Education of Ukraine.

Religious visa
*********************
Single/double/Multiple entry religious visa : each application form should be accompanied by a letter of invitation from a Ukrainian religious
organization which is accepted the State Committee of Religions of Ukraine.

Humanitarian Aid visa
******************************
Single/double/Multiple entry Humanitarian Aid visa : each application form should be accompanied by a letter of invitation from the organization
which is the recipient of the humanitarian aid and letter of support from the Commission for Delivery and Distribution of humanitarian aid
which is a special department of the Cabinet of Ministers of Ukraine (a faxed copy may be accepted),

Transit visa

Double/Multiple entry transit visa : a photocopy of the visa of the country of destination or the travel tickets to non- visa countries should be 
enclosed. A transit visa is valid for 5 days in respect of each entry

Sport and Cultural exchange
*************************************
Single/double entry sport and cultural exchange visa: each application form should be accompanied by a letter of invitation from the relevent
Ukrainian company or organization (a faxed copy may be accepted). The invitation should state full name of the applicant and the purpose
of journey; multiple entry visa: it is obligatory to present a detailed letter of invitation to Ukraine and explain the necessity of issuing 
a multiple entry visa.

*** Nationals of the EU countries, USA, Canada and Japan, Switzerland, Slovak Republic, Turkish Republic who apply for business, private, 
sporting and cultural visas do not require a letter of invitation from Ukraine. It is obligatory to give answers on every question in the application form.

Group visa

application is submitted by post - 10 working days.

***If for some reasons a holder of a visa was unable to travel within the period of its validity, he/she should apply for a new
visa to the Visa Section
